: Acute Necrotising Encephalopathy (ANE), also known as Acute Necrotising Encephalopathy of Childhood (ANEC), is a rare and fulminant encephalopathy, usually triggered by viral infections, characterised by rapid neurological decline and distinctive neuroimaging features, particularly bilateral thalamic involvement. The authors, here present three cases of ANE, one with a rapid catastrophic presentation with shock, seizures and encephalopathy, where the 10-month-old male child had uncal herniation within four hours of admission, the second one involved eight-year-old boy who had acute encephalopathy and seizures, and the third one who was an adolescent female aged 17 years, had a less severe presentation of mild irritability and aphasia. The present case series underscores the pivotal role of Computed Tomography (CT) and Magnetic Resonance Imaging (MRI) in early diagnosis of ANE. The case series also highlights the importance of early, aggressive immunomodulation to achieve favourable outcomes in ANEC. | |||||||
